How to add an in-video form, collect viewer responses, and view results in analytics.

1. Add a form to your video

Open your video, then go to Customize and click Forms

Click then on Add a form.

Choose where the form appears on the timeline, so viewers see it at the right moment during playback.


2. Build your form questions

You can add different question types, including:

  • Text input

  • Multiple choice (including multi-select)

For text questions, you can enable the Email toggle to collect an email address.

For each question:

  • Add the question text

  • Set a property name (used to track answers in analytics)

  • Turn on Required if the viewer must answer before continuing

You can add up to 3 steps/questions in a form and you can add 1 form per video.


3. Publish and preview the form

Click Publish changes to make the form live.

As a viewer watches the video, the form appears at the selected timeline point.
After answering, they can continue watching the rest of the video.


4. View form responses in analytics

Go to your video Analytics to review collected data.

You’ll see submitted values for each form property (for example, trip and email), so you can track and analyze viewer responses.


That’s it! Add a form, publish your video, and start collecting viewer insights directly inside playback.
